What happens first?
Begin with a private intake. After you submit it, you may stop or continue to payment, consent, and appointment selection.
Are visits in person or by telehealth?
New patient and transfer-of-care evaluations are completed in person at our Pompano Beach office. Renewal visits are available by telehealth, by video or telephone.
What does an evaluation cost?
A new patient evaluation is $175. Renewal and transfer-of-care visits are $125. Florida's state MMUR application fee is separate.
Does payment guarantee certification?
No. Payment covers your evaluation. Certification depends on the medical evaluation and Florida requirements.
What should I bring?
Bring photo identification, a current medication list, and any records that may help us understand your history.
What if I am not sure my condition qualifies?
You do not need to interpret the law before starting. Tell us what you would like help with, and we will review it with you.
